Cryotherapy and red light therapy are two of the most requested services in our Recovery Room at Improved Motions Physical Therapy. Patients often ask: “Which one should I do?” The answer depends entirely on what your body needs right now — because these two modalities work through completely different mechanisms.
How Red Light Therapy Works
Red light therapy (also called photobiomodulation) uses specific wavelengths of red and near-infrared light — typically 630-670nm and 810-850nm — to penetrate skin and reach underlying tissues. When these wavelengths hit your cells, they’re absorbed by the mitochondria (your cells’ energy factories), stimulating increased energy production. The results include accelerated tissue repair, reduced oxidative stress and cellular inflammation, increased collagen production, improved circulation, and enhanced nerve function.
Red light therapy is a building process — it helps your body repair, regenerate, and rebuild damaged tissue.
How Cryotherapy Works
Whole-body cryotherapy exposes your entire body to extreme cold for 2-3 minutes. This triggers rapid vasoconstriction — blood vessels constrict, reducing blood flow to the extremities. The effects include rapid reduction in inflammation and swelling, endorphin release for natural pain relief, nervous system downregulation, improved sleep quality, and a rebound vasodilation effect that flushes fresh blood through tissues.
Cryotherapy is a management process — it controls inflammation, calms the nervous system, and creates an environment where healing can happen more efficiently.
When to Use Red Light vs. Cryotherapy
Use cryotherapy when: You’re dealing with acute inflammation (first 72 hours after injury or surgery), post-PT session soreness, acute flare-ups of chronic conditions, or you need nervous system calming for pain management. Cryo is your go-to for anything hot, swollen, or acutely painful.
Use red light therapy when: You’re in the tissue repair phase (after acute inflammation has been controlled), dealing with chronic tendinitis or tendinopathy, recovering from surgery and need to support tissue remodeling, managing skin health or scar tissue, or looking to enhance cellular energy between training sessions.
Use both when: This is where Recovery Stacking really shines. For many patients — especially athletes and post-surgical patients — we prescribe cryo first to control inflammation, followed by red light therapy to stimulate repair. The cryo creates the right biological environment; the red light provides the cellular energy for healing. Different mechanisms, complementary outcomes.
Common Conditions — Which Modality Fits
- Acute ankle sprain: Cryo first 1-2 weeks, add red light starting week 2-3
- Chronic Achilles tendinitis: Red light primary, cryo after PT sessions for soreness
- Post-ACL surgery (week 1-4): Cryo for swelling, red light for incision and tissue repair
- Chronic low back pain: Red light for tissue healing, cryo for nervous system calming on high-pain days
- Golfer’s elbow: Red light on tendon, PEMF as an additional layer, cryo after manual therapy
